探讨分析数字化外科导板精确性的影响因
响因素,以期为数字化外科导板临床实际应用提供理论依据.方法:选取南昌大学附属口腔医院综合科要求种植修复的患者20名,利用SegmaGuideAPP设计数字化外科导板,共26个牙支持式导板,植入35枚植体,其中上颌16枚,下颌19枚.术后拍摄CT,将术后的CT图像重新导入设计软件生成三维模型,与术前模型进行配准重合,利用软件自带的测量工具测量种植体实际位置与设计位置之间的中心轴偏离值,包括顶部偏差.底部偏差.深度偏差和角度偏差.应用SPSS22.0软件进行统计学分析,各组数据均以(均数±标准差)表示,Shapiro-Wilk检验各组数据的正态性,各组间测量值对比采用独立样本t检验,检验水准α=0.05,P<0.05被认为有显着性差异.结果:1.对35枚植体术前术后的位置偏差进行测量分析,其顶部偏差为(1.06±0.52)mm,底部偏差为(1.72±0.64)mm,深度偏差为(0.87±0.28)mm,角度偏差为(3.37±1.27)°.2.上颌与下颌导板精确度的对比,两组顶部偏差分别为(1.42±0.35)mm和(0.75±0.44)mm;底部偏差分别为(2.16±0.46)mm和(1.35±0.53)mm;深度偏差分别为(0.92±0.20)mm和(0.83±0.33)mm和角度偏差分别为(4.45±1.02)°和(2.46±0.53)°,两组(除深度偏差c)之间差异都有统计学意义(P<0.05).结论:数字化外科导板可以获得较高的临床精确性,具有临床应用价值,可用于引导牙列缺损患者的口腔种植修复治疗;上下颌牙支持式数字化外科导板精确性对比,在顶部偏差.底部偏差和角度偏差方面差异有统计学意义,而在深度偏差方面无明显差异.关键字:数字化外科导板;口腔种植;精确度;影响因素ABSTRACTObjective:TheaimofthispaperwastoresearchtheaccuracyofimplantplacementwiththeuseofCAD/CAMsurgicalguidesinoralimplant.Theaccuracyoftoothsupportedsurgicaltemplatewascalculatedandcomparedbetweenthemaxillaryandmandibular,andthefactorsthataffecttheaccuracyoftheCAD/CAMsurgicatemplatewereanalyzed.ThenitcanprovidereferenceforclinicianstousethiskindofdomesticCAD/CAMsurgicalguidetemplate.Method:Therewere35implantswereplacedin20patients.TheSegmaGuideAPPwasusedtosimulateimplantplacementbeforesurgery,andgettingtheCAD/CAMsurgicaltemplatewiththeusingof3Dprintingtechnology.ThepostoperativeCBCTimageswereimportedtoSegmaGuideAPPtogeneratethree-dimensionalmodel,thepreoperativeandpostoperativeCBCTimageswerematchedinthesoftware.Thedeviationsbetweentheplacedpositionandtheplannedpositionweremeasured.ThedatawereanalysedbySPSS22.0software,theShapiro-Wilktestshowedthatallofthedatawerenormality.Themeasuredvaluesofeachgroupswerecomparewiththeindependentt-test,APvalue<0.05wasconsideredstatisticallysignificant.Results:1.Themeandeviationinthepositionatthetopwas(1.06±0.52)mm;themeandeviationinthepositionattheapicalwas(0.87±0.28)mm,thedepthdeviationswas(0.87±0.28)mmandtheangledeviationswas(3.37±1.27)withthe35implants.2.Themeandeviationsinthepositionatthetopwere(1.42±0.35)mmand(0.75±0.44)mm;themeandeviationsinthepositionattheapicalwere(2.16±0.46)mmand(1.35±0.53)mm;themeandepthdeviationswere(0.92±0.20)mmand(0.83±0.33)mmandtheangledeviationswere(4.45±1.02)°and(2.46±0.53)°withthetoothsupportedsurgicaltemplateusedinthemaxillaryanmandibular,respectively(P<.05exceptthedepthdeviation)Conclusions:TheSegmaGuidehashighclinicalguidanceandcanbeusedtoguidedentalimplant;thedeviationatthetop,attheapicalandattheanglewiththetoothsupportedsurgicaltemplateusedinthemaxillarywerehigherthanthatinthemandibular,buthereisnosignificantdifferencebetweenthemandibleandthemandibleinthedepthdeviation.KeyWords:CAD/CAMsurgicaltemplate;oralimplant;accuracy;effectfactorsConclusions:
